Futbolito Soccer Participation Waiver & Release of Liability

By signing this form, I acknowledge and agree to the following on behalf of each child/children named above:

1. Assumption of Risk

   I understand and accept that participation in soccer and physical activity involves inherent risks, including but not limited to minor injuries (such as bruises, sprains, or cuts) and more serious injuries (such as fractures, concussions, or other physical trauma, including, in rare cases, death), as well as emotional stress or fatigue associated with physical exertion.

   
   

   These risks include, but are not limited to, slips, falls, collisions, contact with other participants, equipment failure, or unpredictable acts by other participants, which may result in injuries ranging from minor to serious. These risks exist despite all safety precautions taken by Futbolito Soccer and its staff.

   
   

   I voluntarily and knowingly assume all such risks, both known and unknown, and accept full responsibility on behalf of my child/children.

   
   

2. Medical Release

   I certify that my child/children are in good physical health and able to participate in soccer-related activities. In the event of injury or emergency, I authorize Futbolito Soccer staff to administer basic first aid and, if necessary, seek emergency medical treatment, including transportation to a hospital or medical facility. I understand that I am solely responsible for any medical costs incurred.

   
   

3. Medical Disclosure Policy

   I agree to fully disclose all relevant medical, behavioral, or support needs during registration and also inform staff again before the first session of any updates, including allergies, medications, or unique support needs. I understand that I am responsible for ensuring any necessary medical supplies, such as EpiPens, asthma inhalers, or other required medications, are brought to each session. I understand that failure to disclose relevant medical or behavioral information may affect Futbolito Soccer's ability to provide appropriate supervision or response in case of emergency.

   
   

4. Release of Liability

   To the fullest extent permitted by law, I waive, release, and discharge Futbolito Soccer, its directors, employees, coaches, contractors, volunteers, officers, and affiliated organizations (collectively, the “Released Parties”) from any and all liability, claims, demands, actions, or causes of action for injuries, losses, or damages (whether to person or property) arising out of or related to my child's/children's participation in the program, even if caused in whole or in part by negligence. 

   
   

   I further agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, liabilities, demands, causes of action, damages, losses, and expenses (including legal fees) that may arise as a result of my child’s/children's participation in the program or any breach of this agreement.

   
   

5. COVID-19 / Communicable Illness Acknowledgement

   I acknowledge that participation may involve exposure to communicable illnesses including, but not limited to, COVID-19. I will not send my child/children to class if they are experiencing symptoms of illness. I waive any claim against Futbolito Soccer for illness or related consequences arising from participation.

   
   

6. Behavior & Participation

   I understand that respectful behavior is expected from all participants and accompanying adults. A parent or guardian may be required to participate in sessions for younger children or those requiring additional support. Futbolito Soccer reserves the right to remove a participant or family if behavior compromises safety, staff, or the experience of others.

   
   

7. Inclusivity & Anti-Discrimination Policy

   Futbolito Soccer is a proudly inclusive organization. Discrimination or harassment of any kind, including but not limited to race, ethnicity, gender identity, sexual orientation, religion, ability, or socioeconomic background will not be tolerated under any circumstances. All participants, families, and staff are expected to contribute to a respectful and welcoming environment for everyone.

   
   

8. Photo/Video Release

   1. I understand that Futbolito Soccer may take photos or videos during sessions for use in promotional materials, including print, digital, and social media platforms.

      
      

      If I don't want photos or videos of my child/children in promotional materials, I understand that I must contact the team by email info@futbolito.ca and that Futbolito Soccer will make reasonable efforts to exclude or obscure my child's/children's image in promotional materials.
